1&1 Home Health has been providing excellent home health care services in Southern California. Founded in 2014, 1&1 Home Health is the premiere provider in Orange, Riverside and Los Angeles counties. 1&1 Home Health is endorsed by many prominent doctors for their excellent service. All of our nurses are licensed by the State of California, Medicare Certified and fully accredited by the America Nursing Association (ANA).
What We Do
At 1&1 Home Health, our RN's and LVN's provide prescribed treatment, assess changes and supervise the care of the patient. 1&1 Home Health nursing places emphasis on patient and family education to safely manage care at home as well as promoting patient maximum self-care. Our nurses are on call 24 hours a day, 7 days a week. Our nurses and case managers will arrange and coordinate all needed services. We work directly with our physicans in determining how best to care for our clients. We are always happy to answer any questions our clients and families have.
When to Qualify
All people with Medicare who meet all of these conditions are covered:
- You must be under the care of a doctor, and you must be getting services under a plan of care established and reviewed regularly by a doctor.
- A doctor must certify that you need Home Health Care.
CHF Program
Our program is designed to increase the patient’s knowledge regarding heart failure, thus improving their quality of life and reducing re-hospitalizations. Our program is ideal for patients who have been recently diagnosed with CHF, have recently been discharged from a hospital or rehabilitation facility with a diagnosis of CHF, or are prone to acute and sudden exacerbations of CHF.
Pneumonia Program
Managing patients with pneumonia at home can be less challenging when you have the help of our Pneumonia Home Care program. Our program helps you decide when you can handle the symptoms yourself and when you need our help.
COPD Program
Patients with COPD have many needs when they are at home. Our staff can help you understand how to manage their care safely, as well as provide you a guide for when you need additional assistance. Our COPD Home Care program tells you what you need to know when you are caring for your loved one at home.
